AUDITION FORM

NOTE:
-Webserver system sometimes does not like "auto-populated" field/blank filling.  It prefers actual keystroking the info in.
-You can definitely fill out this form with your smart-phone, but you might find it easier to navigate from a desktop/laptop.
-You will need your audition time in order to submit this form.  Need one? Get one HERE.

    Your Schedule and Conflicts

TheatreMidwest is a professional theatre company, and maintains that environment throughout the process.  All company members are expected to attend all rehearsals, events, meetings, for which they are called.  This could include (but is not limited to):  work calls/load-ins, strike, photo shoots, PR events, costume fittings, or other business.  Therefore it is important that you accurately record all conflicts in the space provided. Casting, assignments, and scheduling will be done based on these conflicts.
Dates, times, nature of conflicting events that occur between OCTOBER 15 AND FEBRUARY 15 that would affect your participation in these shows. (travel, concerts, etc.) --->>>>>
    Regularly occurring conflicts--work, class, etc.

    Your equipment

Coming out of our first season of online theatre-making, we know that the following items are of importance to the success of the process and the product.  We also know that an hours-long rehearsal that involves much talking/dynamic sound and video action takes a much greater toll on one's equipment than a short (or even series of short) meetings or chats-with-friends.  We have no expectations that you are an IT guru (although it's fine if you are!)  None of us are, either.  We collect this information so that, if or when necessary, we can have efficient communication about some potential options if your machine or set-up goes awry during the process.  And it's almost certain that everyone's will at some point (alas).

For better and for worse, this is the way-we-live-now in the theatre, so we kindly ask you to please respond to everything below that you know something about.  Just as "hair color" or "vocal range" or "height" can be factors in casting, so are these items, (for now.)

Please feel free to use the "Other Comments about Equipment?" question at the bottom of this section for any further explanations, things you aren't sure about, etc.
    COMPUTER
What type of computing unit do you plan to use?  Laptop?  Desktop? Few tablets (other than a high end, new-ish, MS Surface) and essentially no phones will work for this on their own.
What system operates or controls that unit?
About how old is this unit?

    MODEM/ROUTER/DATA
Do you know what kind of modem you have? Do you know if your modem is also your router? If they are two different things, do you know what kind of router do you have?
Do you know where and how your modem/router gets shut down and restarted?  And, more importantly, do you have access to it? (We understand that living in an apartment or similar housing, owned by someone else, may limit your access to this gear.)
Can you connect your laptop/desktop *by cable/wire* to your modem or router, thereby not using Wireless/WiFi to get online?
Do you know what kind of service you use for incoming data?  NOTE--a 'hot-spot' via a phone is unlikely to work.

    CAMERA
Is the camera you use for online comms built into computer?  Or externally mounted?
IF EXTERNAL camera, do you know what brand/type/model and/or how old it is?  Is it connected by USB cable or HDMI or other?
Whether built in or attached externally—do you have a good working knowledge of how to adjust camera's settings  in your computer?

    HEADPHONES
Are the listening devices you use for online comms small/in-the-ear 'earbuds' or large/over-the-ear-and-head 'headphones'?  Or do you have both?
Are these listening devices wireless (bluetooth or etc) or 'wired'?
IF WIRED- is it connected by small round (3mm) jack, or USB cable, or Mac/Apple connector, or HDMI, or other?
Whether wireless or wired, do earbuds/phones have the MICROPHONE THAT YOU USE built into them?

    MICROPHONE
Is the microphone you use for online comms built into computer?  Or externally mounted? Or built into your headphone?
IF EXTERNAL mic, do you know brand/type/model, age/how old it is?  And is it connected by small round (3mm) jack, USB cable, or Apple/Mac connector, or Microphone cable (3 prong), or HDMI, or other?

    SOUND CONNECTION
Whether or not you use any wired external sound devices, is your EXTERNAL MICROPHONE "in" jack/hole and your EXTERNAL HEADPHONE "out" jack/hole the *SAME HOLE* or are they *SEPARATE*?  Especially, but not exclusively, on laptops, and probably these are SMALL, ROUND, 3MM jack/hole.
    ONLINE MEETING PLATFORMS
Which of these online programs have you used *successfully* for real-time meetings, streaming, gatherings, rehearsals, etc.?
Which browser software(s) have you used?
    Other comments about *YOUR EQUIPMENT*?
